A new fatality narrative has been published by the Washington Fatality Assessment and Control Evaluation (FACE) Program. The narrative summarizes an incident in which a dairy laborer drowned after the loader he was operating fell into a manure pit. Methamphetamine was found in the operator’s system. Recommendations to prevent similar incidents in the future include surrounding manure storage pits with protective barriers and preventing impaired operators from working.
